To become a transportation services client, click here to download the
application or call RSVP to request an application by mail. Once
an application is submitted, you can begin requesting rides.

Rider Eligibility and Fees:
If you are in need of ongoing or occasional transportation services, this program may meet your needs. Priority for accessing this service will go to those needing rides to in-county medical appointments, but other rides may be approved and will be considered on a case-by-case basis.
Clients of the service:
- Must be a resident of Story County
- Must be willing to pay for the service upon the volunteers’ arrival at your home
- Must be ambulatory, able to get to and from the car with little or no assistance
- Must be willing to fill out a client application and waiver of liability for RSVP to keep on file
- Must be willing to comply with policies regarding criteria for ridership, which will be provided
- Must be traveling within Story County with possible out-of-county exceptions
- Rides must be for 2 or fewer passengers (exceptions may be made at the discretion of RSVP staff)
- Passengers must provide appropriate car/booster seats
- All children under age 18 must be accompanied by parent or guardian
Fees for the service are as follows*:

*RSVP reserves the right to adjust the fee structure as needed without notice.
